[論文新知] BMI較高、眼壓較高

本研究刊登於 Aerosp Med Hum Perform. 2021 Nov 1;92(11):886-897.

分析了39篇文獻

發現BMI每上升1 眼壓上升的風險為1.06倍(95 CI%, 1.04-1.07)

.

當然眼壓高不等於青光眼

但是青光眼的患者需要把眼壓降下來!

題目為 How Does Weight Loss After Bariatric Surgery Impact the Ocular Parameters? A Review

蒐集了23篇研究報告 發現

經過縮胃手術減肥後 血管密度增加、靜脈變細、動脈灌流改善、眼內壓下降

對於長期 視神經纖維厚度 關聯仍不清楚

對於其他眼部不適、維他命缺乏也不清楚

本人不提倡縮胃手術

但是本篇文獻提供了 關於 減肥與青光眼相關性的重要證據

友情與相信是我的救贖、是我得以繼續披著白袍的動力…

我想分享這一段友情的故事、內心的一些觸動

也許能給你一些溫暖、

又或許我們可以為彼此的內心打氣、一起堅強

跟所有的手術一樣、近視雷射有立竿見影的效果

解決病人的需求、卻也有它的短板-

95%可一次達標、達到病人滿意的效果

卻也有5%的機會需要二修或罕見的併發症

.

近視雷射不是救命的手術

救命的手術-那即使有95%的風險，都該冒險🙏

選擇性的手術、也同時因為自費的緣故、自然病人會有很高的期待…

.

往往那5%的機會發生的時候

都不是太平靜…

有的人會認為-一定是做錯了什麼、他才成為那5%

曾經為此陷入了低潮

從那時候開始..我都會再三跟病人確認

「我可以為你拼盡全力、但是仍然有5%無法掌握」

「你確定可以接受這5%的風險、我們才排刀…」

最近有一位好朋友、我們一起在做預防醫學

幫人逆轉三高、擺脫肥胖、重獲無藥人生

她帶她的女兒來、希望能排近視雷射手術

我做了手上所有能做的檢查、確認符合雷射條件

即使到了手術當天

我都再三地跟朋友與女兒確認…

[論文新知] 胰島素阻抗與冠狀動脈阻塞有關

本研究刊登於Caspian J Intern Med. 2023 Summer;14(3):495-506

發現胰島素阻抗與冠狀動脈阻塞(Gensini score, 多少-vessel-disease)有關

而且排除掉確診 二型糖尿、糖尿病前期、BMI>30以上、懷孕或哺乳、腎臟病、心臟衰竭、合併心臟瓣膜疾患 之病患

.

發現Gensini score, 多少-vessel-disease 與胰島素阻抗值 明顯有關

.

Gensini score對於每一條冠狀動脈的分支有不同的權重

這邊我用另外一篇論文(Bangladesh Heart Journal 36(2):113-123)來說明

回到本研究 作者用以下的冠狀動脈攝影來說明他的評分

我們來看研究結果

groupA 是HOMA低於2.5

groupB 是HOMA大於2.5

groupB 的Gensini score, 多少-vessel-disease 明顯較高 而且高出非常多

.

如果我們把每個可能影響的因子都分開來計算

和Gensini score 最相關的是BMI與HOMA p值小於0.001 p值越小越有意義

和多少條血管阻塞(多少-vessel-disease) 最相關的是HOMA p值小於0.001 p值越小越有意義

而是 "位置"

[論文新知] 使用益生菌改善心理健康

本文獻由愛爾蘭學者Timothy G Dinan, Catherine Stanton, John F Cryan發表在 Biol Psychiatry . 2013 Nov 15;74(10):720-6.

整理了2013以前的益生菌與心理健康的研究

這邊我不細講這張圖 以免絕大多數的人都看不懂

簡單來說 他舉的案例是 腸腔好菌會讓免疫反應不要太強 壞菌會讓免疫反應太強 就會讓全身產生壓力反應"Stress"

.

以下我們直接看我覺得比較有趣的重點-人體試驗的相關結果

最下面的healthy volunteer

是法國的研究(目前沒有找到原文 但是有pubmed的摘要 https://pubmed.ncbi.nlm.nih.gov/20974015/)

補充了三十天的Lactobacillus helveticus R0052 and Bifidobacterium longum R0175

進行了以下的評估

"Hopkins Symptom Checklist (HSCL-90), the Hospital Anxiety and Depression Scale (HADS), the Perceived Stress Scale, the Coping Checklist (CCL) and 24 h urinary free cortisol (UFC)."

明顯改善 憂鬱 焦慮 身心症狀...

當然

益生菌這麼多

不可能要每個人去補充每一種菌; 這些研究的重點是 或是一直想強調的是 "我們腸腔的菌叢 是會影響 我們的心情與心理健康的"

怎麼利用飲食 建立好的腸腔環境

先說結論 目前對於髖膝骨關節炎(hip and knee OA)的第一線治療

不是吃藥 不是打針

是教育、適度運動、減肥!

這是本篇論文整理各大醫學會的建議 可以看到最上面三行"教育、適度運動、減肥"!

我們利用本篇論文的圖片來進一步說明

不是定期在關節內增加"潤滑"(例如有人每個月去打玻尿酸 或是以前很流行的吃維骨力) 就能大幅改善的

這是最常見的膝關節X光片 可以看到關節腔狹窄 軟骨磨損 硬骨刺的生成

透過核磁共振MRI的掃瞄 可以看到骨髓的損傷 髕骨後的積水

可以看到發炎的脂肪細胞 也會加重關節的發炎 輪流陷入惡性循環

關節囊的發炎(synovitis)也會加重骨刺的生成

軟骨會有耗損與鈣化的現象

關節周邊的韌帶損傷(ligament damage) 也會和關節囊的發炎 輪流陷入惡性循環

.

當然論文也留了一個小伏筆 在table4

原文是這樣寫的

"Weight loss effective but difficult to achieve and sustain"

就是說

"減肥是很有效的 但是不容易達到與維持"
